Here is a post at PPC Super Affiliate Blog that somewhat describes an issue I had with some ad groups. I had been getting a decent number of clicks, but when I made some changes meant to increase clicks I actually found that the opposite happened. I am not sure if I got kicked into review, but will keep it in mind for future reference.
